Group Benefit Highlights:

Who’s Eligible to Enroll?

Full-time employees scheduled to work at least 30 hours per week are eligible to enroll in medical, dental and vision coverage. New full-time employees will be able to begin enrolling in coverage on the 7th of the month prior to their benefits effective date. For those that enroll, coverage is effective the first of the month following at least 30 days of employment.  Please refer to the Eligibility Chart below to view your enrollment window and when benefits will be effective.

How To Enroll:

On the first day of your enrollment window, you will receive an enrollment email containing log in instructions and benefit details.  Please refer to the Eligibility Chart above to view your enrollment window and when benefits will be effective.

Can I Change My Coverage Anytime?

Once the enrollment window has closed and/ or once benefits are effective, employees are not able to make any changes to their coverage(s) until the end of the year during Open Enrollment or if you’ve experienced a qualifying event that satisfies federal regulations.

Examples of Qualified Life Events

If one of the following qualifying events occurs, you may make changes to your benefits that are consistent with the event.

  • Marriage, legal separation or divorce
  • Birth or adoption of a child
  • Change in eligibility of a child
  • Death of a dependent family member
  • Change in the employment status of your Spouse / Domestic Partner
  • Your Spouse / Domestic Partner reaches age 65 and is covered by Medicare
  • FMLA special requirements
  • HIPAA special enrollment rights
  • Increase or reduction of hours that changes employment status
  • Transfer to or from a benefits-eligible position

PLEASE NOTE:  New elections and supporting documentation must be completed and submitted within 30 days from the date of your Life Event.  To complete the life event process, please contact for further instructions.

Employee Cost of Coverage

Your cost of coverage will vary depending on the plan option and tier you choose.  Medical, dental, and vision health premiums are deducted pre-tax from each paycheck, saving you tax dollars.

IMPORTANT: If you are no longer an active TargetCW employee due to voluntary or involuntary termination, employee coverage terminates on the last day of the month in which the termination occurs. Any owed insurance premiums from prior or current month(s) will be deducted from your final paycheck. Coverage may be terminated for failure to timely pay premiums via payroll.

Aetna Medical

TargetCW offers full coverage EPO and PPO plans through Allied Benefit Systems that use the Aetna network and include prescription drug coverage through CVS Caremark. The plans satisfy the requirement of Minimum Essential Coverage and Minimum Essential Value under the Affordable Care Act.  The plans offer co-payments for doctors’ visits and prescriptions, while also offering an out-of-pocket maximum so employees can have peace of mind in the event of a serious illness or accident.  Health benefits are often complex and can be difficult to understand.  Our TPA, Allied Benefit Systems, will process claims, answer questions, and other important management functions related to our health plans.

Cigna Dental

TargetCW also offers a dental HMO (for CA residence only) and a dental PPO plan through Cigna. Overall good health includes a healthy mouth, gums, and teeth. Gum disease can cause health conditions, like heart disease, stroke, diabetes, and other diseases. Getting regular dental checkups can reduce your risk.  Cigna’s dental plan makes it easier to manage good oral health. Here’s how:

Flexible Spending Account

How they work and why you want to participate

A Flexible Spending Account (125 FSA) allows you to set aside pre-tax dollars from your paycheck to be used for paying out of pocket for eligible health and dependent care expenses which lowers your taxable income and increases your take home pay because contributions you make to your 125 FSA are not reported as income to the IRS.

Eligibility Requirements

  • Enroll by the 1st of the month following 30 days of employment during your enrollment period (or during annual Open Enrollment)
  • Must be an active, full-time worker working a minimum of 30 hours per week

How the Plans Work

You determine the annual amount of your contributions to a plan for the plan year.  When you have eligible expenses, you pay them from your account(s) through your Benefits MasterCard or pay out-of-pocket and submit a claim for reimbursement.

Healthcare FSA

The Healthcare FSA lets you set aside pre-tax funds via payroll deductions. You can use the money to reimburse

yourself for eligible medical, dental and vision expenses.  In 2021, you can contribute up to $1,500.  Any unused funds left in the Healthcare FSA on December 31st of each plan year or after your termination date will be forfeited, except for balances up to $550 that can be carried over into the next plan year, if you’re an active employee and re-enroll in the plan.  To see all eligible expenses, visit:


Dependent Care FSA

This benefit is designed to help you pay for eligible expenses you incur for childcare, or for the care of a disabled dependent, while you work or attend school.  Childcare expenses may include day care, nursery school costs, or after-school programs.  Employees may defer up to $5,000 pre-tax per year.  Funds do not rollover to the next year.


Benefit Card

If you enroll in a FSA, a benefit card will be mailed to you from PlanSource and you will use this card to make payments.  Please remember that if you make a payment and forget to use your Benefits card, be sure to save your receipts just in case PlanSource requests a copy for verification and reimbursement purposes.

  • All receipts should be itemized to reflect what product/ service was paid for and must include the date of service.


  • You can only change FSA amounts during Open Enrollment or within 30 days of a Qualified Life Event.
  • You must be an active worker and re-enroll in the FSA plan each year during Open Enrollment. Your FSA enrollment will not automatically rollover.
  • Any monies left in the Health Care Flexible Spending Account on December 31st of each plan year will be forfeited, except for $550 that can be carried over into the next Plan Year only if you re-enroll.


What Happens to my FSA After My Engagement Ends?

  • Medical and/or Dependent Care FSA claims may be submitted up to 60 days following your termination date, however the dates of service on all claims/ receipts must be prior to your date of termination.  If you do not elect to continue Medical Care FSA through COBRA, any unused funds remaining in your account after your date of termination and/ or after the plan year are forfeited per IRS regulations. 
  • Dependent Care FSA is not eligible to be continued on COBRA as it is not a group health plan.

Commuter Benefits Program

How it works and why you want to participate

TargetCW is pleased to offer our employees this pre-tax benefit for the purchase of various public transportation and commuter products.  Employees nationwide are taking advantage of a federal law designed to encourage the use of public transportation. This law allows employees to reduce their commuting costs by using tax-free dollars from your paycheck (subject to monthly limits) to pay for Mass Transit and Parking fees. Taking public transportation, using vanpools, or bicycling can not only save you money but it also dramatically reduces your environmental impact. Commuter benefits can help you do your part in an easy and low-cost manner!


How the Plan Works

  • You determine the monthly contribution amount to a plan. When you have eligible expenses, you pay them from your account(s) through your Benefits Mastercard or pay out-of-pocket and submit a claim for reimbursement.
  • When you enroll for transit or parking benefits, TargetCW will deduct the total amount pre-tax from your first or second paycheck of the month after enrollment. Deductions will always be made one month in advance so it’s like pre-paying for the benefit.
  • Our Commuter program is also compliant with specific city ordinances like Seattle and San Francisco. Click on the city name for more information.


Commuter Benefits Program Highlights

  • The IRS max pre-tax limits per month for 2021 are $270 for transit and $270 for parking.
  • You can elect both transit and parking benefits up to $270 by filling out a separate enrollment form for each one.


Benefit Card

  • Within 10-14 days after enrollment, you will receive a debit card from PlanSource to the address listed on your Commuter Benefits Program Enrollment Form. Funds will be loaded onto this card for both transit and parking expenses.  If your transit provider or parking facility does not accept debit card payments, you may pay out of pocket and submit a reimbursement request online or by using their free, mobile app on your phone, My Benefits Accounts!
  • If you also have a Flexible Spending Account with TargetCW, your funds will be placed on the same debit card for easy access.

General Notice Of COBRA Continuation Coverage Rights

**Continuation Coverage Rights Under COBRA**


This notice is applicable if you obtain coverage under a group health plan (PanAmerican Medical, PanaBridge Dental/Vision, and/or MetLife Dental). This notice has important information about your right to COBRA continuation coverage, which is a temporary extension of coverage under the Plan. This notice explains COBRA continuation coverage, when it may become available to you and your family, and what you need to do to protect your right to get it. When you become eligible for COBRA, you may also become eligible for other coverage options that may cost less than COBRA continuation coverage.

The right to COBRA continuation coverage was created by a federal law, the Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA). COBRA continuation coverage can become available to you and other members of your family when group health coverage would otherwise end. For more information about your rights and obligations under the Plan and under federal law, you should review the Plan’s Summary Plan Description or contact the Plan Administrator.

You may have other options available to you when you lose group health coverage. For example, you may be eligible to buy an individual plan through the Health Insurance Marketplace. By enrolling in coverage through the Marketplace, you may qualify for lower costs on your monthly premiums and lower out-of-pocket costs. Additionally, you may qualify for a 30-day special enrollment period for another group health plan for which you are eligible (such as a spouse’s plan), even if that plan generally doesn’t accept late enrollees.

What is COBRA continuation coverage?

COBRA continuation coverage is a continuation of Plan coverage when it would otherwise end because of a life event. This is also called a “qualifying event.” Specific qualifying events are listed later in this notice. After a qualifying event, COBRA continuation coverage must be offered to each person who is a “qualified beneficiary.” You, your spouse, and your dependent children could become qualified beneficiaries if coverage under the Plan is lost because of the qualifying event. Under the Plan, qualified beneficiaries who elect COBRA continuation coverage must pay for COBRA continuation coverage.

If you’re an employee, you’ll become a qualified beneficiary if you lose your coverage under the Plan because of the following qualifying events:

  • Your hours of employment are reduced, or
  • Your employment ends for any reason other than your gross misconduct.

If you’re the spouse of an employee, you’ll become a qualified beneficiary if you lose your coverage under the Plan because of the following qualifying events:

  • Your spouse dies;
  • Your spouse’s hours of employment are reduced;
  • Your spouse’s employment ends for any reason other than his or her gross misconduct;
  • Your spouse becomes entitled to Medicare benefits (under Part A, Part B, or both); or
  • You become divorced or legally separated from your spouse.

Your dependent children will become qualified beneficiaries if they lose coverage under the Plan because of the following qualifying events:

  • The parent-employee dies;
  • The parent-employee’s hours of employment are reduced;
  • The parent-employee’s employment ends for any reason other than his or her gross misconduct;
  • The parent-employee becomes entitled to Medicare benefits (Part A, Part B, or both);
  • The parents become divorced or legally separated; or
  • The child stops being eligible for coverage under the Plan as a “dependent child.”

When is COBRA continuation coverage available?

The Plan will offer COBRA continuation coverage to qualified beneficiaries only after the Plan Administrator has been notified that a qualifying event has occurred. The employer must notify the Plan Administrator of the following qualifying events:

  • The end of employment or reduction of hours of employment;
  • Death of the employee;
  • The employee’s becoming entitled to Medicare benefits (under Part A, Part B, or both).

For all other qualifying events (divorce or legal separation of the employee and spouse or a dependent child’s losing eligibility for coverage as a dependent child), you must notify the Plan Administrator within 30 days after the qualifying event occurs. How is COBRA continuation coverage provided?

Once the Plan Administrator receives notice that a qualifying event has occurred, COBRA continuation coverage will be offered to each of the qualified beneficiaries. Each qualified beneficiary will have an independent right to elect COBRA continuation coverage. Covered employees may elect COBRA continuation coverage on behalf of their spouses, and parents may elect COBRA continuation coverage on behalf of their children.

COBRA continuation coverage is a temporary continuation of coverage that generally lasts for 18 months due to employment termination or reduction of hours of work. Certain qualifying events, or a second qualifying event during the initial period of coverage, may permit a beneficiary to receive a maximum of 36 months of coverage.

There are also ways in which this 18-month period of COBRA continuation coverage can be extended:

Disability extension of 18-month period of COBRA continuation coverage

If you or anyone in your family covered under the Plan is determined by Social Security to be disabled and you notify the Plan Administrator in a timely fashion, you and your entire family may be entitled to get up to an additional 11 months of COBRA continuation coverage, for a maximum of 29 months. The disability would have to have started at some time before the 60th day of COBRA continuation coverage and must last at least until the end of the 18-month period of COBRA continuation coverage. Second qualifying event extension of 18-month period of continuation coverage

If your family experiences another qualifying event during the 18 months of COBRA continuation coverage, the spouse and dependent children in your family can get up to 18 additional months of COBRA continuation coverage, for a maximum of 36 months, if the Plan is properly notified about the second qualifying event. This extension may be available to the spouse and any dependent children getting COBRA continuation coverage if the employee or former employee dies; becomes entitled to Medicare benefits (under Part A, Part B, or both); gets divorced or legally separated; or if the dependent child stops being eligible under the Plan as a dependent child. This extension is only available if the second qualifying event would have caused the spouse or dependent child to lose coverage under the Plan had the first qualifying event not occurred.

Are there other coverage options besides COBRA Continuation Coverage?

Yes. Instead of enrolling in COBRA continuation coverage, there may be other coverage options for you and your family through the Health Insurance Marketplace, Medicaid, or other group health plan coverage options (such as a spouse’s plan) through what is called a “special enrollment period.” Some of these options may cost less than COBRA continuation coverage. Health Care Reform

TargetCW is dedicated to helping employees navigate new federal guidelines mandating insurance coverage beginning January 1, 2014. The changes are meant to help expand access to adequate and affordable health care coverage; Health Insurance Marketplaces (an online exchange) where individuals can shop for health insurance coverage will be operated by each state or, if the state does not have its own marketplace, by the Federal government. As part of the Affordable Care Act, workers with household incomes between 100% and 400% of the federal poverty level may be eligible for subsidies to help offset health insurance costs when purchased through their state Health Insurance Marketplace. See Below for 2015 federal poverty levels.
